5k Family Fun Run

We are so excited to bring to our community a 5K Run/Race!  This event will serve as a fundraiser for our 7th Annual Back To School Rally!  One of our amazing staff members has been training and running 5K races and she brought this lovely idea to me!  This will be a fun event for ALL Ages!  Each participant will receive a t-shirt, metal, and refreshments!  We will also have prizes!  All ticket purchases and sponsorships are tax exempt, we really appreciate you for supporting this amazing cause!

Photo/Video Waiver:

This document permits Sevynteenth and their respective partners including the Cleveland Metroparks, the irrevocable right to photograph, video-record, use, reproduce or broadcast the name, image and voice of myself, or son/daughter as well as the work, conversations, photographs and sounds of my son/daughter and anything related with these, in all forms of communication now known or hereinafter invented in any territory, without time restrictions, for any purpose including live or recorded television, cable, radio or theater, in print or in the form of publicity or commercials for marketing or promotion of any kind in connection with programs in which my child is participating, has participated or will participate. In addition, I authorize the aforementioned same rights with respect to my name, image, voice, conversations and works of art related with the programs of Sevynteenth. I waive all rights to inspect or approve final products, including written or spoken copy, which may be created in connection with said programs. No claim will be made by me on behalf of my child or depends upon my child. No other representations have been made. In consideration of this guaranteed consent, my son/daughter will have the opportunity to participate in the above mentioned programs. I have the right to sign this letter of consent on behalf of my son/daughter (or ward) listed above.

Injury/Liability Waiver:

“I am the parent or legal guardian of the Participant (s). I have read this document, and I am signing it freely. I understand the legal consequences of signing this document, including (a) releasing the organization from all liability on my and the Participant’s behalf, (b) waiving my and the Participants’ right to sue the organization, (c) and assuming all risks of Participant’s participation in this Activity, or any events incidental to this Activity. I allow the Participant to participate in this Activity. I understand that I am responsible for the obligations and acts of the Participant as described in this document. I agree to be bound by the terms of this document.”
